Muay Thai, also known as Thai Boxing, is a traditional martial art and combat sport originating from Thailand. To enhance the effectiveness of our training, this course includes mixed-gender sessions. Physical contact between male and female participants may occur during practice, such as assisted movements or partner drills.If you feel uncomfortable with physical contact with the opposite gender, please inform the on-duty coach before the session begins. This allows us to make appropriate arrangements and avoid any misunderstandings.We respect every participant’s preferences and are committed to providing a safe, respectful, and inclusive training environment.  To ensure your safety and the quality of training, please complete the following health declaration before the session begins. If you have any past injuries or underlying medical conditions, you may also choose to inform the on-duty coach privately. All information will be kept strictly confidential.*Please indicate if you have experienced any of the following:*- Heart disease or arrhythmia- High or low blood pressure- Fractures, dislocations, or major sports injuries- Respiratory conditions (e.g., asthma)- Neurological disorders (e.g., epilepsy)- Diabetes or other chronic illnesses- Recent surgeries or hospitalizations- Medications that may affect physical performance- Any other health concerns that may impact trainingIf you prefer not to disclose these details publicly, please speak with the on-duty coach before class. *1. Liability Waiver*
I understand and acknowledge that participation in the Martial Arts Club’s Self-Defense Class (hereafter referred to as “the Class”) involves physical activity, including but not limited to fitness training, technique practice, and simulated combat. By signing this document, I confirm that:


- I voluntarily choose to participate in the Class and accept all associated risks;
- I am physically fit and capable of engaging in the activities involved;
- I release the Martial Arts Club, its instructors, staff, and affiliates from any liability for injuries, accidents, or loss of personal property that may occur during or as a result of the Class;
- I agree to follow all safety instructions and conduct myself responsibly during training.


I understand and acknowledge that participation in activities or training sessions organized by this organization may involve physical risks, including but not limited to injury, accidents, or even death. I confirm that I am in suitable health to participate and voluntarily accept all associated risks.This organization, including its coaches, staff, and venue providers, shall not be held liable for any injury, property loss, accident, or death that may occur during the course of the activity. No compensation or legal responsibility will be assumed.I have read and fully understood the contents of this disclaimer and voluntarily agree to its terms by signing below.

*2. Non-Refundable Policy*
I acknowledge and agree to the following payment and refund terms:


- All fees paid for the Class are *non-refundable*, regardless of personal circumstances such as absence, withdrawal, or schedule conflicts;
- Once registration is confirmed, no transfers, postponements, or substitutions will be accepted;
- In the event of cancellation due to unforeseen circumstances (e.g., natural disasters, government restrictions), the Club may offer make-up sessions or alternative arrangements, but refunds are not guaranteed.


*3. Confirmation*
By signing below, I confirm that I have read and understood the terms outlined above. I agree to comply with all rules and accept full responsibility for my participation.

一、免責聲明*
本人明白並同意參加拳會自衛術班(下稱「本課程」)可能涉及身體活動,包括但不限於體能訓練、技術練習及模擬對抗,存在一定風險。本人確認:


- 自願參加本課程,並了解所有活動性質及潛在風險;
- 本人身體狀況良好,適合參與相關訓練;
- 在課程期間如發生任何意外、受傷或財物損失,拳會及其教練、工作人員不承擔任何法律責任;
- 本人將遵守所有安全指引及教練指示,並承諾不進行任何危及自身或他人安全的行為。

本人了解並同意參加本會所舉辦之活動或訓練課程可能涉及身體風險,包括但不限於受傷、意外甚至死亡。本人確認自身健康狀況適合參與該活動,並願意自行承擔所有相關風險。本會及其教練、工作人員、場地提供者不會就任何因活動期間所發生之傷害、財物損失、意外或死亡承擔任何法律責任或賠償。本人已詳細閱讀並理解本免責聲明內容,並自願簽署以示同意。


*二、不可退款條款*
本人明白並同意以下收費及退款政策:


- 所有課程費用一經繳交, *概不退還* ,無論因個人原因缺席、中途退出或其他情況;
- 課程名額有限,報名成功即視為確認參加,拳會不接受任何形式的轉讓或延期;
- 如因天災、政府政策或其他不可抗力因素導致課程取消,拳會將視情況安排補課或其他替代方案,惟不保證退款。


*三、確認聲明*
本人已詳細閱讀並理解上述條款,並同意遵守所有規定。本人簽署此文件即表示接受所有內容,並放棄對拳會提出任何法律追究的權利。



*Typhoon & Severe Weather Notice*颱風及惡劣天氣通告*

In response to typhoon or severe weather warnings issued by the Hong Kong Observatory, the following arrangements will apply:- When Typhoon Signal No. 8 or above, or Black Rainstorm Warning is in effect, all classes and activities will be *suspended or cancelled*;- If weather conditions improve, classes will *resume* when it is deemed safe. Please stay tuned for updates;- Cancelled classes will *not be rescheduled or refunded*. We appreciate your understanding;- Students are advised to monitor weather updates and stay in contact for the latest arrangements.
